Social Prescribing

Have you heard about Social Prescribing?
The purpose of the service is to work together with your GP or other medical staff to support you if you feel you may have any underlying social issues that are affecting your mental health or general mood.


Some of the issues that the service can support with whilst working with your medical team are:

  • Social Issues affecting physical or mental health
  • Social Isolation
  • Bereavement
  • Domestic Abuse
  • Housing Issues
  • Employment/Training
  • LGBTQ+
  • Caring Responsibilities
  • Drugs/Alcohol
  • Pain Management

  • The next step will be to make an appointment with the Social Prescribing Link Worker (Louise). You can do this directly with the reception team. Your GP or Medical Practitioner may also suggest you could benefit from an appointment.


    At your appointment, Louise will be able to take the time to listen and together work out what is important to you and devise a plan to help you. Referrals or signposting will then be made to the most suitable services or groups that could benefit you the most. These services may include statutory groups (e.g. counselling services) along with local community interest groups.


    Whilst you’re waiting to attend your chosen service or within the first few weeks of attending the services, further appointments will be offered to you with Louise. This will help to decide if the service you have been referred to continues to be suitable or whether further referrals are needed.

    Travel Immunisations / Vaccinations

    If you think you may need travel vaccinations, please consult an Patchs form which will be assessed my a member of the nursing team. We will contact you to advise whether vaccinations are needed, and where applicable to book your appointment.

    Post Natal Care & Child Health Surveillance

    The practice offers pre-booked appointments with a GP for both a postnatal check for new mothers and a 6-8 week check for babies.

    The health visitors team are based at Meadowside. They offer a baby clinic on Thursday mornings 09:30 – 11:30 (drop in facility) and child health development sessions.

    Minor Surgery

    The practice is able to offer appointments for Minor Surgery, to remove minor skin lumps and lesions, provide joint injections etc., please contact the practice for further details.

    You will have to be seen by a GP in the first instance for assessment.
