You are welcome to register with
the Practice if you live within the
Practice catchment area.
Our receptionists will be happy to
assist you and it would help if you
could give us your medical card and
proof of identity and residence
(i.e. a passport).
You will be offered a new patient
check with our Healthcare Assistant
to enable us to take note of your
current health status. It is very
important for you to attend. Please
make every attempt to come for this
check up. If you cannot attend,
however, you should let us know and
arrange to make another appointment
as soon as possible.
Registering With The Practice
Registration forms and a
pre-registration questionnaire need
to be completed and handed into
reception. Please pick up a form at
reception to complete.
Alternatively, the pre-registration
forms can be accessed from the New
Patient Questionnaire link below.
All new patients must have a new
patient check within the first six
weeks of being accepted.
